New Hampshire’s rates of skin cancer are well above the national average, and the co-founder of ConvenientMD urgent care hopes to increase access to the care needed to help prevent it.
Max Puyanic partnered with Ben Collins, who served as chief growth officer at ConvenientMD, to open Optima Dermatology & Medical Aesthetics, which has been in the works since both left the company in 2020 to find solutions for shortages in dermatologists. More providers are retiring than graduating from residency programs.

The word eczema can be used as a generic type of skin rash. There are actually multiple different types of eczema, and it’s important to know which type you have because they all have different symptoms, characteristics, and different approaches to treatment and management.
Starting With the Basics: What is Eczema?
Eczema is a very common, non-contagious skin condition that causes dry, itchy, red, and often bumpy patches on the skin. It can be quite uncomfortable but is manageable with the right treatment.
Eczema, which is a type of dermatitis, is actually the name for a group of different conditions, and each manifest differently and can require different treatments. Eczema causes the skin to lose its ability to act as a barrier, which initiates an inflammatory response. Without the barrier function, your skin is left open to irritation, dryness, or infection.
What Are the Most Common Types of Eczema?

Eczema manifests in different ways, and depending on which type you have, the best treatment for eczema can vary.
Here Are 7 Types of Eczema:
Atopic Dermatitis (AD)
Atopic Dermatitis begins with itchy skin that forms a rash. It affects infants, children, and adults. In adults, AD often manifests on the hands or eyes as dry, scaly, or patchy areas. Common symptoms include itching, and raw, bleeding, or oozing skin.
Contact Dermatitis
Caused by an irritant that comes in contact with your skin and triggers an itchy, rashy reaction. In some cases, the cause is as simple as the laundry detergent you’re using, but it can also be more difficult to know what you are coming in contact with that is irritating your skin.
Dyshidrotic Eczema
Also called pompholyx, this type of eczema causes tiny, highly itchy blisters on the hands and feet and can become chronic. The most effective treatment is identifying the allergen that is triggering the blisters.
Hand Eczema
Deep, painful cracks on the hands accompanied by excessively dry or chapped skin. It can lead to painful, bleeding blisters that are uncomfortable and hard to manage, and traditional hand creams and moisturizers do little to relieve the symptoms.
Neurodermatitis
Highly common among adults and tends to affect only one or two patches on the body, as opposed to other types of eczema that affect multiple areas at once. The cause of Neurodermatitis is unknown, but treatments are highly effective.
Nummular Eczema
Typically brought on by extremely dry skin, Nummular Eczema manifests as itchy, red bumps and can look similar to Atopic Dermatitis and Ringworm. Moisturizing treatments are the most helpful for preventing Nummular Eczema.
Stasis Dermatitis
Primarily affects the lower legs and feet, causing inflammation, swelling, itchiness, scaling, and hyperpigmentation. In some cases, ulceration occurs. Stasis Dermatitis is caused by chronic edema or swelling, and treatment is highly focused on a self-care routine.
Which Types of Eczema are Most Common?
Atopic dermatitis (AD) is the most common type of eczema, affecting about 16.6 million U.S. adults, according to the National Eczema Foundation.
Contact dermatitis is also quite common, affecting about 1 in 5 people simply because it’s brought on by contact with an irritant. Think of how many potential irritants and allergens your skin comes in contact with on a daily basis. From a necklace chain to body wash, there is a high likelihood that something will cause a contact dermatitis reaction on your skin in your lifetime.
What Are the Different Treatment Options for Each Type of Eczema?
Although there are several different types of eczema, they are all treatable using a combination of medicated solutions and self-directed care, such as avoiding allergens and triggers.
Everyone is different, and eczema can affect different people in different ways, and with varying levels of severity. Only by working with your board-certified dermatologist can you identify a treatment plan that will appropriately address your specific type of eczema and symptoms.

What made you decide to pursue a career in dermatology?
After college, I worked in New York City as a dermatology medical assistant and loved it! This was my first exposure to the field of dermatology, and it sparked my interest in medicine. Coincidentally, also during my time in New York, I developed contact allergies and underwent extended patch testing. I was grateful for the specialists who solved the mystery and significantly improved my quality of life. Collectively, these experiences inspired me to attend medical school and solidified my specific passion for dermatology.
What is your number one tip when it comes to taking care of your skin?
Always, always wear sunscreen! Rain, shine, snow, clouds, in cars, indoors, and outdoors. Pick a moisturizer with SPF 30 or higher and apply it to your face, ears, neck, and the backs of your hands EVERY DAY.
What is your favorite aspect of being a dermatologist?
I love the variety in dermatology. I interact with patients of all ages and see a range of conditions from skin cancer, and acne, to preventive care, surgery, cosmetics, and more. The days are never boring!
How would you describe your approach to treating patients?
My approach to dermatologic care involves attentively listening to patients and tailoring treatment plans to meet their individual needs and goals. I really enjoy connecting with patients on a deeper level, sharing in the joy of their successes, hearing their stories, and learning about them as a person.
What is one thing you want your patients to know about you?
I feel a deep connection to my patients because of the care I received as a dermatology patient. I understand what it’s like to have a skin condition that requires advanced diagnosis and specialized treatment. I draw upon this experience when evaluating patients and designing individualized care plans to meet their goals.
What do you like to do when you are not in the office treating patients?
I always enjoy reading a good book, antiquing, exercising, and taking in the natural beauty and ample history all around us in New Hampshire! Most recently, I have really enjoyed doing home projects and renovations with my husband, Tom.

Where did you go to college?
I attended Bethel College (now University) in Indiana for my undergraduate degrees, then went to Case Western Reserve University for my first master’s, and finally, I attended the University of Mount Union’s PA program.
What made you decide to pursue a career in dermatology? What did the path to get there look like?
In dermatology, you need to be able to play detective and investigate the problem with the patient, which I really enjoy. I also enjoy the hands-on component of dermatological procedures such as biopsies and excisions. My dermatology training after school took place at Cleveland Clinic where I worked with amazing PAs and physicians who helped me develop a deeper understanding of the specialty.
On your journey to becoming a board-certified physician assistant, what was the best advice you were given?
Going into PA school, I was pretty confident I wanted to work in dermatology, but my professors were vocal about the importance of being open to working in any specialty. Learning about many of the specialties made me realize there are a lot of crossovers between them and it’s important to know medicine outside of your own specialty to better help your patients.
What is your number one tip for taking care of your skin?
Always prioritize sun protection.
What is your favorite aspect of dermatology?
I enjoy the relationship I develop with my patients. It’s great to see someone long-term and see how they improve.
How would you describe your approach to treating patients?
I like the visit to feel like a conversation rather than a one size fits all approach. It’s important to consider things like a patient’s lifestyle and accessibility, and you can only find that information out through a conversation. I also think education is very important – I find it’s easier to continue a treatment plan if everyone is on the same page as to why it’s important.
If there is one thing you want your patients to know about you, what would it be?
I want my patients to feel like they are contributing to their health during our appointments. If you feel like something isn’t working for you, let me know and we’ll figure it out together!
What do you like to do when you are not in the office?
I enjoy spending time with my family and being outside. I also really enjoy traveling, reading, and baking.

Isotretinoin is a medication used to treat severe cases of acne, in most cases cystic or stubborn acne that has not responded to other well-known acne treatments.
This medication works by targeting the sebaceous gland itself, where it works to slow the production of sebum (the oil secreted by these glands) and to make it less sticky, reducing the chances of clogged pores and acne.
Dr. Gregory Delost, practicing at the Optima Dermatology facility in Mentor, Ohio, and catering to the Greater Cleveland area, offers comprehensive insights into the transformative potential of Isotretinoin for patients enduring the challenges of severe acne.
Who is a Candidate for Isotretinoin Therapy?
Patients with severe nodulocystic acne or even mild or moderate acne that hasn’t responded to conventional treatments are excellent candidates for isotretinoin. The bottom line is that any patient with acne scarring potential should be considered.
How Does Isotretinoin Work to Stop Acne?
The major factors that cause acne involve the skin not being properly shed, increased sebum (oil) production, and bacteria feeding on the oil to cause inflammation.
Isotretinoin targets all three causes by shutting down the sebaceous glands and the bacteria which depend on the oil to cause inflammation. The medication also helps normalize proper skin shedding, which cuts down on comedones (blackheads and whiteheads) formation.
What Side Effects Should I Expect When Starting Isotretinoin?
Dry lips and dry mucous membranes (eyes, lips, nose). Luckily, these pesky side effects are temporary, and we have ways to control them through the treatment course.
Additionally, we can see elevations in blood lipids, such as triglycerides, which are routinely monitored during treatment. The elevations are temporary and resolve at the end of the treatment course. Far less common side effects include muscle aches after strenuous exercise and headaches.
How Long is a Patient Typically on Accutane?
Every patient is different, but the average patient will take 20-24 weeks to reach the treatment goal. Sometimes, we can get there quicker if the patient is able to tolerate the dryness from higher daily doses.
Would you prescribe isotretinoin to your own family members?
Yes. In fact, I have personally taken the medication. As a teenager, I struggled with acne and found success with isotretinoin after trying and failing every other treatment option out there.
Seeing first-hand how skin disease can have such a profound psychosocial impact on the quality of life, especially on a teenager, and how much my confidence improved with clear skin, it motivated me to become a dermatologist and do the same for others.

Where did you go to college?
When I was in high school in Youngstown, OH, I knew I wanted to be the first physician in my family. I had the great opportunity to get into a dual acceptance program at Gannon University and Lake Erie College of Osteopathic Medicine. Other than all of the snow, it was a great educational experience.
What made you decide to pursue a career in dermatology?
When I was in high school I had really bad skin and acne; I tried and failed just about everything. When I was treated with Accutane, it was life-changing for me and probably the reason I am working as a dermatologist today. I went from always staring at the ground to making eye contact with others. Now I want to help my patients gain confidence through healthy, beautiful skin.
What is your number one tip when it comes to taking care of your skin?
I’ll give you my top three tips that have the most science to back them up:

If there is one thing you want your patients to know about you, what would it be?
There is a huge psychosocial burden of skin diseases. Some of the diseases we deal with may not be life-threatening, but they severely affect quality of life. My goal is to make patients happier, more productive members of society.
What is your favorite aspect of being a dermatologist?
Every day is totally different. In one day, I see the whole continuum of life ranging from babies to patients more than 100 years old. I see a combination of medical dermatology – treating acne, psoriasis, eczema, rosacea, autoimmune blistering conditions, hair loss, rashes, and other complex skin disorders; surgical dermatology – treating skin cancer, cysts, and other skin growths; and aesthetics – helping patients to simply love their skin more through neuromodulators, fillers, and other cosmetic procedures. I truly never get bored and I learn something new every day.
What is your approach to treating your patients?
The original translation of the word “physician” is “teacher,” so first and foremost I want to be a teacher to my patients and share insight and knowledge about different skin conditions. Gone are the days when doctors talked to their patients in a more paternalistic way, as now medicine is more of a collaboration between physician and patient. Nowadays, patients are more educated than ever, so it’s my job to navigate them through all the information and misinformation. Most importantly, it is my mission to treat my patients as if they are my own loved ones.
What do you like to do when you are not in the office treating patients?
I love to spend time with my wife, Rachel, and our dogs, Hugo and Fitzpatrick. We love to work out, golf, cook/bake, and support our Cleveland teams. Additionally, I am a huge watch nerd.

Where did you go to college?
Miami University in Oxford, Ohio for my undergraduate degree, and then Case Western Reserve University School of Medicine in Cleveland, Ohio for medical school. I then completed an internship/residency at the University Hospitals of Cleveland, the Cleveland VA Medical Center, and the MetroHealth Medical Center.
What made you decide to pursue a career in dermatology?
I liked the opportunity to be specialized in a field where I could care for young through old patients and really make a visible difference in their lives.
What is your number one tip when it comes to taking care of your skin?
Sun protection, including sunscreen, hats, and sun protective clothing. These measures will prevent skin cancer and keep you looking your best as you get older!
What is your favorite aspect of being a dermatologist?
Making a difference in my patients’ lives – from improving confidence with clearer skin in a teenage acne patient to diagnosing and helping treat melanoma in an elderly patient, and everything in between. I love getting to know my patients and their whole family. I have had some patients now for 20 years!
How would you describe your approach to treating patients?
I try to take care of patients as I would like my family and friends to be cared for.

Where did you go to school? Did you know you wanted to focus on dermatology from the start?
I went to Harvard Medical School. I had no idea I wanted to focus on dermatology until I did my rotation!
Why did you choose dermatology? What did the path to get there look like for you?
I really enjoyed all the specialties I rotated through – from medicine to surgery to the emergency room. I had a mentor in my third year on my medicine rotation who told me I should consider dermatology. I listened to him and did a dermatology rotation. I absolutely loved it – in dermatology, you are the expert. I love the visual nature of the field and I love how specialized it is.
In terms of practice, the diversity and breadth of dermatology are amazing. I get to see patients of all age ranges and practice medically, surgically, and procedurally. I am fortunate enough to treat medical conditions like acne, psoriasis, and eczema, which often can be very challenging to patients and require close medical management. I also get to work closely with patients in skin cancer identification from skin checks and treatment, whether that be surgical or medical.
In addition, dermatology has a close association with cosmetics. Helping people look and feel their best is very rewarding! As a dermatologist, I can help patients medically, surgically, and procedurally, which is really rewarding as we can reach for many different options when designing a treatment plan.
What is your number one tip when it comes to taking care of your skin?
It has to be sunscreen. So many skin cancer and cosmetic concerns could be prevented with regular sunscreen use. Look for broad-spectrum SPF30+ (I prefer 45+) sunscreen. The best kind is the one you will actually wear. My favorites right now have been tinted mineral-based (zinc oxide or titanium dioxide ) sunscreens.
What is your favorite aspect of dermatology?
While skin is superficial, skin diseases are anything but! Seeing a patient’s confidence and quality of life improve as we’re able to help them is so rewarding.
What is your approach to treating your patients?
I strive to really understand a patient’s goals to work as a team to create a treatment plan that works best. While we have evidence-based options for certain conditions, there is no one-size-fits-all approach. Understanding patients’ goals for treatment and bringing my expertise to the table helps us create a plan that works best.
What do you like to do when you are not in the office?
I love being outside and spending time with those close to me. You can catch my dog Ella and me hiking, camping, surfing, and traveling often.
If there was one thing you wish your patients knew about you, what would it be?
I am here to help! Skin exams or conditions can often feel embarrassing but they don’t need to be. I will always try to make you feel as comfortable as possible so please feel free to bring anything up.

Chelsea graduated from Simmons College with a Bachelor of Science in Nursing, and went on to receive her Masters of Science in Nursing in 2015. How and when did you know you wanted to be a Nurse Practitioner? Were you always interested in aesthetics, and what was your path to get here?
I was one of those kids whose dreams were all over the place. One week I would want to be a Disney princess and the next an astronaut. It wasn’t until I started to look at colleges in high school that I realized I wanted my life’s work to embrace my strengths of compassion and hard work with the desire to make lasting changes in the world. I have always had an innate desire to help people become the best version of themselves and pursuing a career as a nurse practitioner was the perfect way to do just that.
I spent the first five years working in Gastroenterology (GI) at Beth Israel Lahey Health, where I helped heal various GI ailments; mainly focusing on using food and exercise as medicine.
The ability to assist patients on their journey to better their own health, sense of self, and overall well-being from the inside out was incredibly fulfilling. Naturally, I was inspired to pursue a career in aesthetics and I was elated to find a career path that combined my lifelong desire to help others in a medical setting that inspires positivity and artistry. I firmly believe that aesthetic medicine epitomizes the “art of nursing” because it enables me to fulfill my creative side while still using my clinical judgment.
What is your number one tip when it comes to taking care of your skin?
Without a doubt, sunscreen! Wearing sunscreen is one of the best and easiest ways to protect your skin’s appearance and health at any age. When used regularly and consistently, sunscreen helps prevent sunburns, premature aging, and most importantly skin cancer.
What do you enjoy most about treating your aesthetic patients and what is unique about your approach?
There is nothing more gratifying than comparing before and after pictures with a patient. Photos are truly the foundation for communicating results because images and visuals often describe far more than words alone can. The result is rarely as impressive as the journey toward the result—before and after photos convey that journey perfectly. My approach to aesthetic medicine is unique because I pride myself on having the ability to interpret patient goals and skillfully tailor a treatment approach to provide an optimal aesthetic outcome, and in the process, cultivate a trusting clinician-patient relationship.
For someone who is interested in, but new to new to medical aesthetic treatments, how do you recommend they get started?
I would strongly encourage them to simply book a consultation. Our skin is the biggest organ we have and it deserves some pampering! The aesthetic world can be intimidating and consultations facilitate an open and honest conversation about specific concerns, budget, treatment options, and expectations. Consultations are the key to developing a customized treatment plan that will deliver measurable results!
Where are the most common areas you treat with dermal filler?
One of the most common areas I treat with dermal filler is the cheeks. It is also one of my favorite areas to treat because it is an effective and conservative way to correct aging. Loss of volume is a hallmark sign of aging, which starts in the mid-face—specifically, the cheeks. Midface volume rejuvenation with dermal filler directly improves the appearance and contour of the cheeks but also has beneficial effects on neighboring areas of concern including the undereye, nasolabial folds, lines around the mouth, and marionette lines. Facial contouring with cheek filler is truly the key to restoring the triangle of youth!
For microneedling, which patients would you recommend this treatment for, and what should patients expect?
From reducing the appearance of wrinkles to improving the appearance of acne scars, pigmentation, skin texture, and large pores—microneedling is a versatile and effective treatment that can fulfill a diversity of needs. Microneedling, also known as collagen induction therapy, is a process involving repeated puncturing of the skin with tiny, sterilized needles. Carrying out precise, controlled “micro-injuries” leads to skin rejuvenation and the treatment of fine lines, skin laxity, scarring, pigmentation, cell turnover, and increased collagen production through the process of dermal remodeling.
Patients can expect mild short-term swelling, redness, and skin flaking, which can last a few days post-procedure. Most people can return to wearing regular makeup within a day of treatment. A series of three to five monthly treatments are generally recommended to achieve desired results.
What do you like to do when you are not treating patients?
When I am not in the office treating patients, I love to spend time with my husband and our two dogs. Being born and raised in New England, we love to be outside no matter what the season! You can always find us outdoors—at the beach, hiking the White Mountains, or simply relaxing in the backyard.

Q: Where did you go to school? Did you know you wanted to focus on dermatology from the start?
A: Marietta College in Ohio. I was always interested in dermatology, but it wasn’t until my clinical rotations during PA school that I fell in love with the specialty and realized there was no other specialty that I wanted to work in.
Q: Why did you choose dermatology? What did the path to get there look like for you?
A: I chose dermatology because I enjoy the variety of services that it offers. On a typical day I am seeing patients from all ages with conditions ranging from acne to melanoma. I also enjoy the mix of clinical time with procedures. Every day in the office is different. I love knowing that I’ll never get bored with the unexpected nature of what each new day will bring.
During PA school I chose to do my elective rotations in dermatology with Butler Health System in Slippery Rock, PA and Dr. Wagamon in Warren, OH. I fell in love with the specialty during my time spent on these rotations and started applying for dermatology PA jobs while in school. I was lucky enough to be hired by Butler Health System where I developed the skills and knowledge I have today. I’m excited to continue growing and learning as a dermatology PA during my next chapter with Optima Dermatology in Mentor.
Q: What is your number one tip when it comes to taking care of your skin?
A: Wear sunscreen and visit a dermatologist for annual skin exams. Prevention and early intervention is much easier than trying to fix years of damage!
Q: What is your favorite aspect of dermatology?
A: There are so many aspects of dermatology that I enjoy, but I think one of my favorites is the ability to get patients 100 percent clear from their conditions – whether that be skin cancer, psoriasis, acne, hair loss, etc. Through the incredible medications and medical interventions available, many of the issues in dermatology can be completely treated and patients’ lives are transformed.
Q: What is your approach to treating your patients?
A: I like to work collaboratively with my patients to diagnose and treat them in a way that makes them feel comfortable and confident. I never would want my patients to leave a visit questioning a diagnosis or being uncomfortable with a treatment plan, so I strive to ask a lot of questions and form connections with each one of my patients.
Q: What do you like to do when you are not in the office?
I love to spend time with my family. My husband Brock and I just welcomed our first baby in April. I also have a golden retriever who I love like a second child. I also enjoy traveling, spending time outside, and relaxing after a long day with some Netflix and a good glass of wine!
Q: What is one thing you want your patients to know about you?
A: I think it’s really important for patients to remember that their doctors and healthcare providers are people just like them. I may not know everything and have all the answers right away, but I will always do my best to help each one of my patients and treat them the way I’d want my family to be treated.
